The subtitle of Dr Miller's textbook underscores the physiologic foundation on which surgical practice is based, and this third edition delivers a compendium. I agree with the observation in the forward that too often students and residents recite protocol rather than have an understanding of the biology. This work seeks to fill the gap both for the student of surgery and for the professor. I have always felt that the onus is on teaching physicians and surgeons to be able to elaborate on the physiology and pathophysiology of health and disease. These chapters allow such a discussion stemming from the premise that surgical disease arises from derangement of normal physiology. Compared with the earlier additions, the third edition offers added breadth with associate editors and many new authors.
