Teaching Hospital Status and Operative Mortality in the United States

Hayanga et al1 assessed mortality following colon resections as reported in the National Inpatient Survey, concluding that outcomes were worse at teaching hospitals (TH) compared with nonteaching hospitals (non-TH). Although this finding was statistically significant, review of the statistical methods used to arrive at this conclusion suggests that inadequate risk adjustment resulted in an erroneous conclusion.

Teaching hospitals provide care for higher-risk patients than non-THs. Because of their training mission, THs accept patients with advanced-stage or complex diseases that non-THs generally do not want to provide care for because of the greater time and expense associated with the clinical care these high-risk patients need. Given this reality, when comparing outcomes between these very different clinical care environments, a patient's baseline risk must be accounted for.
