This book, an accumulation of experience by three authors who have performed more than 5,500 subcortical stereotaxic operations, including 3,500 for Parkinson's disease, is divided into four principal sections. The first portion is a detailed anatomic, physiologic, pathologic, and biochemical review of the extrapyramidal motor system and the regulatory influences of the basal ganglia, thalamus, substantia nigra, and subthalamic pathways on this system. The authors include a review of the relevant experimental animal data concerned with the aforementioned concepts of motor function mediated by the extrapyramidal system. The pathologic changes in these regulatory nuclei and pathways are discussed with the purpose of integrating pathologic findings with the clinical symptoms and signs in patients with Parkinson's syndrome.
The second section is a description of 17 autopsy cases after stereotaxic lesions. The authors carefully document the preoperative abnormal neurological findings with the results after stereotaxic surgery, and correlate the precise anatomic location
