Error in Byline, Author Affiliations, and Author Contributions.  In the “Image of the Month” article by Khalaileh et al, published in the October 2009 issue of the Archives (2009;144[10]:975-976), the fourth author should have been listed as Samir Abu-Gazala, MD, in the author byline and as Abu-Gazala in the Author Affiliations, and Author Contributions. This article was corrected online.
