Dr Zikria and colleagues, in a book titled Reperfusion Injuries and Capillary Leak Syndrome,1 attribute the increased capillary permeability after injury to reperfusion injury and/or inflammation. Certainly, many sick patients retain fluid and have interstitial edema as they progress from SIRS to multiple organ dysfunction syndrome to multiple organ failure. This is an unsolved and difficult clinical abnormality.
