Assessment of splanchnic perfusion has been a desired goal because of the known sensitivity of the splanchnic vascular bed to hypoperfusion associated with systemic shock and low cardiac output states. Indeed, splanchnic hypoperfusion has been postulated to be central to the pathogenesis of the multiple organ dysfunction syndrome, which remains the leading cause of death in the surgical intensive care unit.1 For example, "stress-related" gastric mucosal injury has been associated experimentally with a local ischemia-reperfusion injury and generation of reactive oxygen species.2
