While we agree that cases need to be individualized and that multiple factors are important to healing, proper flap selection is a significant determinant of pressure sore treatment success. As outlined in our discussion, several issues influence the preference for one flap over another for any given site, including flap design, primary healing rates, and the location of a flap relative to the site of coverage. Individual flaps work better in one location compared with another primarily owing to the arc of rotation (eg, the tensor fascia lata flap is better suited for trochanteric coverage than for ischial coverage.)
