He attended the University of Pennsylvania, Philadelphia, where he also completed his medical studies (1837), and at the same time served as a private student of William Horner (1793-1853), professor of anatomy and dean of the school. Following graduation, Smith took a position as resident physician of Pennsylvania Hospital for 2 years. Believing his medical background to be incomplete, he spent 1840 and 1841 in Europe. Intending to make surgery his specialty, Smith returned to Philadelphia in the fall of 1841, where he began to teach private classes in minor surgery.
