We would like to thank Adelman and Chelcun for their valuable comments on our recently published article in the Archives.1 We absolutely agree with the call for a multimodal approach aimed at improving the current patient safety protocols that are apparently insufficient in protecting our patients from preventable harm.1- 2 This notion is supported by a very recent publication presenting evidence that preventable patient harm from medical care continues to be highly prevalent and has not improved (despite extensive efforts in the realm of patient safety) in response to the groundbreaking Institute of Medicine report on medical errors in 1999.3
